NHS Trusts Partner With MySense To Reduce Hospital Admissions and Support Wellbeing At Home
Three health and care organisations join forces to support vulnerable people at home.
Reducing the need for reliance on hospital care is one of the key goals for our work with the University Hospitals of North Midlands NHS Trust, Stoke-on-Trent City Council and Midlands Partnership NHS Foundation Trust.
The three organisations have come together with MySense to monitor early changes in health and behaviour at home for the region’s most vulnerable patients.
The High Intensity Users Team are using MySense’s AI led insight to look for patterns of change and areas of concern which if addressed promptly can help reduce further hospital admissions at Royal Stoke University Hospital and County Hospital, Stafford.
Helen Ashley, Director of Strategy at University Hospitals of North Midlands, said:
“The aim is to identify, intervene and resolve any issues or challenges the patients are experiencing to reduce the need for reliance on hospital care and associated risks, maintaining health and wellbeing, while promoting independence and choice for patients.”
When the insight leads to some form of intervention the High Intensity Users Team contact ‘key responders’ who could be family, a GP, carer or the Community Rapid Intervention Service.
