This new section in our app will help to track subtle, emerging changes, allowing for preventive care and action

The app now offers insights into an individual’s health, sleep, bathroom and activity, and is supported by the launch of our new help centre with health-and-wellbeing related articles.

Almost 1/10 unplanned hospital admissions last year were for people with pre-existing conditions. Of those patients, 48% could have avoided emergency hospital admission with earlier interventions. This costs the NHS almost £20bn p/a and, more importantly, hospital stays reduce people’s independence.

In most cases, a lack of insight robs us of opportunities to take corrective action sooner — to introduce what’s known as Positive and Proactive Care, which if deployed could prevent or delay premature declines in health and wellbeing, reducing the risk of outpatient care, hospitalisation, nursing home admissions, and deaths.

With our new app release we’re offering actionable insights to help understand emerging care needs. Depending on their circumstances, these insights could see an individual change their living environment, seek medical advice or seek informal or formal care. Those with already established care needs can use the data to inform changes at the next level of support, increasing responsibility for carers or perhaps even changes to medication or treatments.

We’re offering the opportunity to have more positive, empathic interactions. By presenting the data in the way we do, we stand by our commitment to offer personalised insights. The data is compared and presented against the individuals’ ‘normal’ habits rather than what is expected of them by society. It’s absolutely fine if someone usually goes to bed at 2am and wakes late in the morning, it’s only when it differs (and is combined with other factors) that we alert them and carers to some unusual activity. It’s also made obvious in the charts and graphs when there is a change in behaviour.

We’re tracking subtle, emerging changes, allowing for corrective action sooner. Long term and short term trend analysis is critical for getting ahead of escalating decline. A decline or an inability to do one thing can often lead to other activities being compromised. For example, poor sleep over consecutive nights could make a frail person less stable on their feet, which in turn could make them less confident getting up and down from their chair, which would have an impact on how frequently they were eating or drinking, leading to dehydration and potentially culminating in a Urinary Tract Infection. A domino effect. We’re looking to minimise the impact of this by giving visual prompts to introduce proactive care that is less aggressive, less intrusive or severe, and therefore more likely to be accepted, complied with, minimising the disruption to a person’s life and preserving their sense of dignity.
